The short version
Semax is a synthetic peptide seven amino acids long, built by grafting a stabilizing tail (Pro-Gly-Pro) onto a fragment of the hormone ACTH. In animal research it rapidly increases the brain's own growth-support proteins — BDNF and NGF — in specific regions, and in stroke models it seems to protect brain tissue and preserve memory by shifting immune and vascular gene expression [2][3][4]. It reaches the brain quickly when sprayed into the nose, though it breaks down just as fast [6].
Here is the honest part. Semax is not FDA-approved for any use. It is a prescription drug only in Russia and Ukraine — registered there for stroke recovery and cognitive impairment — and everywhere else it is an unscheduled research chemical. Nearly all the mechanistic evidence is from rodents, and almost all human clinical work was done in Russian institutions, published in Russian-language journals, and never independently replicated in the West [2]. Community reports of mental clarity, sharper focus, and verbal fluency are genuinely consistent and worth reading — but they are anecdotal, not clinical evidence. What it is
Semax (full sequence: Met-Glu-His-Phe-Pro-Gly-Pro, often written MEHFPGP) is a synthetic linear heptapeptide derived from the ACTH(4-7) fragment of adrenocorticotropic hormone, extended at its C-terminus with a Pro-Gly-Pro tripeptide that slows enzymatic breakdown and prolongs its biological activity. It is commonly supplied as the acetate salt.
Two points of precision matter here. First, Semax lacks the steroidogenic activity of full-length ACTH — it does not stimulate cortisol release, even though it shares part of ACTH's sequence. Second, Semax and Selank are strictly distinct compounds: different sequences, different primary mechanisms, different research endpoints, and no meaningful pharmacological overlap despite their shared Russian origin and a PGP tail they happen to share. In Russia and Ukraine, Semax is a registered prescription nasal spray used in acute stroke, transient ischemic attack, and cognitive-impairment settings. In the United States and EU it is not approved for any human indication; it is classified as an unscheduled research chemical.
How it works
Semax's best-characterized mechanism in rodent brain is rapid, region-specific upregulation of the neurotrophins BDNF and NGF. A single 50 microgram-per-kilogram intranasal dose in male Wistar rats raised NGF and BDNF mRNA in the hippocampus and raised BDNF mRNA in the brainstem and cerebellum, while NGF mRNA fell in frontal cortex — a pattern that suggests the peptide does not simply flood the brain with neurotrophins but shifts their balance in a topographically specific way [3]. The same lab found that Semax also binds a specific, reversible, calcium-dependent receptor site in rat basal forebrain (KD ~2.4 nM) and that BDNF protein in that region rose rapidly and selectively three hours after dosing [4].
A second documented mechanism is inhibition of enkephalin-degrading enzymes — the peptidases that normally break down the brain's own opioid peptides. In human serum in vitro, Semax inhibited these enzymes at an IC50 of approximately 10 micromolar, more potently than standard reference inhibitors; this is thought to prolong endogenous opioid-peptide signaling [7].
In cerebral ischemia models, a genome-wide transcriptional analysis after permanent middle-cerebral-artery occlusion in rats showed that Semax's protective effect is dominated by shifts in immune-system genes (immunoglobulins, chemokines — more than half the affected genes) and vascular-system genes (24 at three hours, 12 at 24 hours), rather than a single receptor action [2]. The peptide's neuroprotection is thus framed as immunomodulatory and pro-vascular, not simply neurotrophic.
Pharmacokinetically, radiolabeled Semax reached rat brain within two minutes of a single intranasal dose, with about 0.09% of the administered radioactivity per gram appearing in brain tissue as approximately 80% intact peptide and the rest as metabolites — followed by rapid enzymatic degradation of the intact heptapeptide [6]. The implication is a fast onset and a short duration of effect, which aligns with what community reporters describe.
What the research shows
Neurotrophin upregulation. In male Wistar rats, a single intranasal dose produced region-specific increases in NGF and BDNF gene expression within hours, with BDNF mRNA rising in hippocampus and BDNF protein rising in basal forebrain — findings replicated across multiple experiments by the same group [3][4].
Neuroprotection in ischemia. After permanent MCAO in rats, Semax shifted the transcriptional landscape toward immune and vascular regulation, characterizing its protection as a broad anti-ischemic program rather than a single-pathway intervention [2]. In a focal photoinduced prefrontal-cortex ischemia model, six days of intranasal Semax decreased infarct volume and preserved conditioned passive-avoidance memory [5].
Spinal cord injury. In a 2025 study in female C57BL/6 mice with T9-T10 spinal cord injury, Semax improved locomotor recovery across three behavioral measures and reduced pyroptosis (inflammatory cell death) via an Oprm1/USP18/FTO deubiquitination axis — an effect on the mu-opioid-receptor gene that complements the enkephalinase-inhibition data [1].
Enkephalinase inhibition. In human serum in vitro, Semax inhibited neprilysin-type enzymes that degrade endogenous enkephalins (IC50 ~10 micromolar), more potently than comparative reference inhibitors; shorter and longer peptide fragments lost activity, identifying a length-dependent structure-activity relationship [7].
What the research does not show. There are no published randomized controlled trials in Western populations, no rigorous controlled human trials for ADHD or age-related cognitive decline, and essentially no independent replication of the Russian clinical literature in FDA or EMA jurisdictions. Extrapolation from rodent models to healthy human nootropic use is not established.
Reported effects, cautions & safety
The following account of real-world community reports is drawn from nootropic forums, biohacker writeups, and peptide-user communities. These are anecdotal reports, not clinical evidence. They describe subjective experience, not controlled outcomes, and should be read with that distinction firmly in mind.
What people commonly report as benefits:
- Fast, clean mental clarity. The single most consistent description across positive accounts is a quick-onset sense of clear-headedness — mental fog lifting without the wired or racing quality of a stimulant. The word "clarity" appears in almost every positive write-up, and many users specifically contrast it with caffeine by calling it "focus without the jitters."
- Sustained task-focus. People describe staying locked onto a task for longer — ending a workday with a genuine sense that more got done. The effect is described as quiet productivity rather than a euphoric push.
- Stimulant-free motivation. Many users describe motivation and physical energy rising for several hours without feeling overstimulated. A minority say the lift is genuinely energizing all day; others stress it is subtle and only obvious in hindsight.
- Improved verbal fluency and word recall. A distinctive recurring report: conversation gets easier, vocabulary feels "more extravagant," and words come faster. Some users single out the N-acetyl variants as particularly pronounced for verbal fluency.
- Mood lift and stress resilience. A modest acute mood bump and a more even-keeled feeling are commonly described, with some accounts of anhedonia lifting or negative rumination reducing. This is reported as a steadying effect, not a high.
What a meaningful minority reports:
- Subtle or no effect. Many people say Semax is "not a nootropic you feel," that any benefit is very subtle and only obvious in hindsight, and some report essentially nothing at all. A subset describe feeling flat or slightly depersonalized. Expectations of a dramatic effect are frequently disappointed.
- Short duration. The noticeable effect lasts only a handful of hours before tapering, which is consistent with the rapid clearance seen in pharmacokinetic data [6]. Many redose later in the day to extend the window.

Key safety cautions from the literature:
- Unregulated supply. In the US, Semax is sold only as an unscheduled research chemical with no required testing of identity, purity, or sterility. Research-grade material differs from pharmaceutical-grade product.
- Limited independent long-term data. Almost all human experience and clinical evidence originates from Russian and Ukrainian practice; there are no FDA or EMA-approved indications and no published Western randomized controlled trials [2].
- Neurotrophin effects are powerful and not fully understood. Rapidly driving BDNF and NGF expression or shifting large banks of immune and vascular genes in a healthy brain over months is not a trivial intervention, and long-term consequences in healthy non-clinical users are uncharacterized [3][4][2].
- Drug interaction unknowns. In rodents, Semax raised the serotonin metabolite 5-HIAA and strongly potentiated amphetamine-evoked dopamine release without raising baseline dopamine on its own. In human serum it inhibited enkephalin-degrading enzymes. These profiles make additive or unexpected effects with stimulants, serotonergic drugs, or opioid-active agents a genuine but unstudied concern [7].
- No established human dosing framework. All quantitative dosing in the literature is from animal experiments expressed per kilogram of body weight. There is no validated human dosing schedule outside Russian pharmaceutical formulations.
Where it fits in cognitive-nootropic research
Among the two peptides on this desk, Semax is the lead and the more research-dense — with a specific receptor-binding result, a clear neurotrophin-upregulation mechanism, multiple ischemia-model datasets, and a consistent community-reported nootropic signal. Its story is also the more complicated one: that signal rests almost entirely on Russian preclinical work and Russian clinical tradition, and it has essentially never been independently validated in a Western controlled trial.
Contrasted with Selank, Semax is the more cognitively forward of the two — its primary endpoints in research are neuroprotection, learning, and memory, whereas Selank's primary endpoint is anxiolytic and its cognitive benefit is largely a downstream consequence of quieting background anxiety. They share a PGP tail and the same ethnic-origin story, but the mechanisms, the target populations in research, and the subjective quality of effects people report are categorically different.
